In consideration of being allowed to participate, I hereby assume any risk (both known and unknown), take full responsibility for, and waive any and all claims for personal injury, including severe bodily injury or death, relating to my child's participation with Rise High Elite, LLC, including participation in practices, games, travel, and unrelated events. I hereby agree to forever release any and all claims against, and to indemnify and hold harmless Rise High Elite, LLC (including any of its players, coaches, officers, board members and volunteers) for any injury to my child, even if caused by the negligence of Rise High Elite, LLC (including any of its players, coaches, officers, board members and volunteers). I understand that participation in practices, games, and other events and activities with Rise High Elite involves risks, including permanent disability or even death, and I agree to assume all risks, both known and unknown.
